designed to identify the root causes for a failure and the
actions necessary to prevent recurrence in future. It helps
professionals determine what happened, how it happened and
why it happened. FMEA can be considered a proactive way to
avoid failures that have not occurred before. RCA is
generally initiated when a failure has occurred. Failure
Analysis (FA) refers to the understanding and categorization
of a failure, not necessarily to find the root causes that
